Periodontal Cleanings (not healthy gums) | $200
- The 12 month membership fee is due in full upon joining
- Save 5% on your initial membership fees when you join as a family
- Membership is effective the 1st of the month of which payment is received
- It is the sole responsibility of the member to maximize their benefits within the 12-month membership period. If the appointments are not used, the member will not be entitled to a refund, and unused cleanings will not be rolled over into the following yearÂ
- Renewal payment is due the beginning of the same month each year.
- The anniversary month of your membership is the month that you originally joined, regardless of when payment is received.
Membership is limited to allow for a maximized Dentist to Patient time, ensuring a Truly Unique Dental Experience!
How does the membership work?
For a flat annual fee, you and your entire family will receive:
- Two Dental Cleanings (healthy gums)
- Unlimited Oral Exams
- Peridontal Evaluation
- Oral Cancer ScreeningÂ
- Unlimited Digital X-rays
- Intraoral Camera Exam
- Fluoride Treatment
- Periodontal Disease Care (up to 3)- $25 per visit
*Fees are based on the usual and customary fee at the time of service.
What are the exclusions?
- Our membership is NOT a dental insurance plan, and can only be used here at Dental Care Team
- It's non-transferable
- It cannot be combined with any other dental insurance coverage
